Scope of Services:
The Jefferson Comprehensive Health Center has an adult medicine department headed by qualified medical professionals. We offer referrals, medicine assistance, diabetic counseling and other programs.
As a medical home, we provide comprehensive, evidence-based, primary care and preventive medicine for all ages, from infants to seniors, including:
- Routine physical exam
- Sport or work physical exam (MDOT physicals)
- Immunizations
- Cancer screening
- Acute illnesses and minor injuries
- Chronic disease management
- Self-management support
- Vaccinations
Availability: All JCHC health centers
Quality care is our focus. Clinical outcomes for preventive measures and management of chronic diseases are evaluated and reported to ensure optimal care. These include management of diabetes, hypertension, high cholesterol, as well as immunizations and cancer screening.
